A brow lift, also referred to as a forehead lift, is performed to eliminate the appearance of deep lines and furrows on the forehead. In many instances, a brow lift is performed in combination with a facelift or eyelid surgery / blepharoplasty in order to achieve a balanced, natural-looking result to facial rejuvenation surgery. We will be able to determine if you are a good candidate for a brow lift during your consultation at our practice.
During the brow lift procedure, one of several different approaches will be taken to allow Dr. Gaines to reduce forehead wrinkles and reposition the eyebrows. Often, if forehead wrinkles are not excessively deep, the brow can be lifted through a series of three very small incisions along the hairline that allow for release and lifting of the forehead tissues with the surgical endoscope. In addition to this Endoscopic Brow Lift, other approaches include an open incision across the top of the head (within the hair), or an open incision along the hairline on the forehead and temple. The final surgical plan will be decided upon by the patient and Dr. Gaines during the preoperative consultation visit.

Otoplasty / Ear Re-Shaping
Otoplasty, also referred to as ear re-shaping or ear pinning, is performed to adjust the shape and prominence of a person’s ears. This surgery is particularly helpful for those with ears that stick out far from the head. Otoplasty can be performed on its own or, if needed, in combination with another procedure. We will be able to determine if you are a good candidate for otoplasty or perhaps another facial rejuvenation procedure during a consultation at our practice.
During the otoplasty procedure, the cosmetic surgeon will place a small incision behind a patient’s ear. The surgeon will re-shape and adjust the contours of the underlying cartilage. In some cases, the surgeon will remove cartilage and then proceed to reshape the ear. The surgeon may also use an alternate otoplasty technique or make incisions in a different area depending on the patient’s unique situation.

Structural Fat Grafting / Lipostructure
Structural fat grafting, also referred to as lipostructure, is one method we use at our practice to treat facial wrinkles or enhance the volume of certain areas of the body. Unlike other dermal fillers and injections, structural fat grafting makes use of a patient’s own body fat from a donor site on the body, limiting the possibility of complications or allergic reactions following treatment. The common donor sites for lipostructure include the buttocks, thighs, and abdomen. We will be able to determine if you are a good candidate for the procedure and the best donor site for the procedure during a consultation at our practice.
During lipostructure, fat is extracted from a pre-determined donor site on a patient’s body and is injected into the treatment area. In the face, this technique is most frequently used along with a facelift to strengthen or soften different facial features, or it can be used as an alternative to a facelift when a more subtle result is desired. When used in this way, structural fat grafting is an excellent technique for augmenting the appearance of the lips, the jaw line, or the cheekbones. Lipostructure can also be used to fill in contour abnormalities or depressions associated with scars or wrinkles, often in combination with wire subcision, tightening the overlying skin and minimizing their appearance.

Often used in combination with structural fat grafting techniques, wire subcision is a procedure that allows for the release of tethered scars or very deep wrinkles through tiny skin punctures. Innovated by a Russian plastic surgeon in 1991, wire subcision devices are available today under the name Wire Scalpel® and SurgiWire®. A very fine braided wire, attached to a needle at both ends, is passed beneath the skin at the level of the tethering, and is used to divide any attachments between the undersurface of the skin and the deeper tissues of the face. Once these attachments have been severed, fat grafts are often placed between the skin and deeper tissues to prevent their re-formation. This technique is particularly useful for diffuse acne scars, depressed scars of any type, and very deep wrinkles not treatable by BOTOX® injection or fillers alone.
